I have been working in self-management for the past 10 years, primarily working for the Expert Patients Programme. Initially I worked in North East London as a Senior Trainer, introducing the programme and supporting Primary Care Trusts and Voluntary organisations to build their capacity and embed the programme within their areas. During this time I delivered many tutor trainings to people where English was their second language and feel I am very able to ensure my trainings are inclusive.

I continued working for the Expert Patients Programme once it became a community interest company initially as the Business Development Manager and then the Regional Manager for London.

Mark has over 20 years' experience as a facilitator and trainer. He has worked in a variety of roles and contexts within the health and social sectors, in Britain, wider Europe and Southern Africa. Mark has post-graduate degrees in Sociology (MA with distinction), Research & Evaluation (MSc) and an MPS in counselling and ethics.

Much of his work has focused on organisational impact and effectiveness. He provided support for the development of Living with a Long-term Illness Project and helped to facilitate the development of the quality assurance system (Stepping Stones to Quality, SS2Q) for the Expert Patients Programme.

Jane Cooper-Neville is one of the founding Directors of Talking Health Network Ltd. She has 20 years multi disciplinary experience of working within the Third Sector and NHS. She has been at the forefront of the development of lay led self-management for people living with long-term conditions for the past 15 years.

She was Director of Self-Management at the Long-term Medical Conditions Alliance and was responsible for leading the introduction of the Chronic Disease Self-Management Course (CDSMC) to the UK voluntary sector.

She led the development of Ss2Q, a cross sector quality assurance framework and self-assessment tool for lay led self-management, and the creation of the Quality Institute for Self Management Education and Training (QISMET).

Jane has a first class degree in Health Social Science and an MSc in Medical Sociology. She has a Certificate in Transactional Analysis Counselling. In January 2004 her contribution to the ongoing development of the knowledge base for lay led self-management was recognised with the award of an Honorary Research Fellowship from Coventry University.

As a Psychology Graduate in 1978, I moved abroad for ten years, working in agriculture and Teaching English as a Foreign Language for the British Council. During this time I discovered the Alexander Technique and Tai Chi, which opened my eyes to the possibility of change and freedom from lifelong asthma. And so my long journey into self-management began. On return to the UK, I trained as an Alexander Technique Teacher and Tai Chi Teacher and have been teaching both disciplines continuously since 1992.

I worked for three years for Rethink (previously NSF) and Mind and I was an early passionate convert to the Recovery model, delivering Anxiety Management, and Mindfulness Based Stress Reduction courses.

I came across CDSMC in 2002, and was inspired by the simplicity and effectiveness of the programme. I joined EPP in 2004 as a Trainer, where I stayed for over seven years. My roles included working with PCTs to manage all aspects of contract and course delivery. I assisted in the collation of the Stepping Stones to Success Supporting documentation and I was Eastern Regional Lead for ‘Training and Quality' to Ss2Q standards. I am accredited to Assessor Trainer level.

Catherine Cross has over twenty years' experience of leading and delivering complex and successful transformational change programmes for both Central and Local Government. Ian was originally trained as a classical singer and worked as a performer and teacher for 25 years before a mid life crisis led him to complete the Graduate Membership of the CIPD. From there he joined Arthritis Care in 2001 and began his involvement with self management.

Initially trained by Ian McNeil as a Master Trainer in ASMP and CDSMP he furthered his involvement by being trained as a T Trainer at Stanford in 2007, returning the following year to train on the new Type 2 Diabetes programme. He is now trained on PSMP too.

Over the last 5 years he has run over 30 leader trainings and helped set up programmes in Ireland, Lithuania and Slovenia. He is the current chair of the UK T Trainer group and works closely with EPP and other organisations delivering self management training.

External trainings have included running two Master Trainer Trainings mostly for staff from English PCT's and he has also run Assessor, Delivery Skills and Update Training as well as working as an independent facilitator. He has been involved with SS2Q for a number of years and also has experience in the video conference delivery of courses to remote communities.

I have lived with a long term health condition for over ten years. I worked for over 40 years in the audit and accountancy fields across both commerce and industry. My volunteering roles included Leader and Trainer of Leaders in the Scout movement. I have also worked as a volunteer with Mind on a community theatre project.

Since 2010 I have volunteered with the NHS Expert Patient Programme delivering over 40 CDSMC courses in Greater Manchester. I am a Tutor, Assessor and Apprentice Lead Trainer. During my career at British Airways I managed front line staff in the delivery of consistently high levels of service, building relationships and gaining loyalty of staff and customers. I was a key element in their customer service training department and am skilled in coaching and developing people in effective behavioural skills.

Joining EPP as a Senior Trainer enabled me to develop my passion for empowering people living with long-term health conditions, who often feel excluded, to take control of their health and better manage their lives. I have been fortunate to work within the field of self care and self-management for over a decade. It has personally transformed parts of my life and I have been moved by the countless others who have thrived from the principles and the culture that it fosters.

I held the role of Self Management Programme Manager and Self Care Lead in Public Health within the NHS and local authority in Dudley. In that time I developed the programme from the NHS EPP pilot stages, through embedding the service and developing a provider, commissioning and local training and quality assurance function that was widely regarded as an example of best practice and accredited by QISMET under their QIS2012 standards.

Over the years I progressed from being a participant on a course in Birmingham to recently qualifying as a T. Trainer with the pleasure of training the lead trainers of the future, it has been a long journey and I feel the experience I have gained at each stage has been so valuable. Undertaking my master training at Stanford University was an experience I will never forget however it is the individual participant's journeys of transformation that stay with me and keep me hooked on the programme.

Dudley has made the decision to fully commission the programme externally so it's time for me to move on. I am continuing to work within the public health field as programme manager and looking forward to further adventures in self-management as a consultant through the Talking Health Network.

An experienced Operational and Change Manager, with a background in local government. My specialisms include the set up and management of contact centres, including the first local government contact centre to deliver Adult and Children's services, The 80-seat centre answering approx 900,000 multi service contacts per annum and was the first public sector contact centre to achieve Call Centre Association (CCA) accreditation.

Jane Harris has fifteen years' experience as a consultant in social care and health. I was supporting people to live the lives they want despite the labels they end up with for several years when I heard about self-management for the first time in 2007. My career had spanned working with people with learning disabilities and mental health issues in both direct support roles and in marketing and business development roles for charities and social enterprise.

I became involved in the roll-out of Staying Positive - the self-management programme for young people as part of EPPCIC in 2007, going on to manage the National Register of Self - Management and latterly supporting QISMET - the Quality Institute for Self - Management Education and Training with their communications and marketing.

I have a first class honours degree from the London School of Economics in Industrial Relations and HRM and a diploma in Voluntary Sector Management from Cass Business School.

I run my own shiatsu therapy business supporting people to connect with their own health and well-being and facilitating change through therapeutic touch. I am passionate about this work and the potential for a changing and dynamic relationship with our own health.

I also run a small marketing consultancy specialising in supporting third sector organisations and small businesses to make sense of marketing "nonsense" and create plans that work for them and are actually implemented.

I have previously owned and run my own business and worked for many years as a lecturer in further education before joining the NHS in 1995.

I have worked as PA to several Chief Executives within the NHS and as Complaints Manager for several years before finally moving into the management of the Expert Patients Programme when the trusts in Worcestershire merged and the post became countywide. I oversaw the merging of the three county programmes and developed the programme over several years until it became a stand-alone programme with 25 volunteer tutors, 8 Assessors and 2 Lead Trainers.

As Programme Manager I developed close contacts with neighbouring Trusts and assisted them in building their EPP capacity by training, supervising and assessing their tutors. I developed close working links with GPs and other clinicians/support workers across Worcestershire and with the local University where myself and a Lead Trainer delivered Self-Management modules on the pre-registration nursing programme. I have organised and hosted training and supervision events for tutors, assessors and Lead Trainers from other parts of the country, supported by Talking Health.

I am an Accredited Tutor and Assessor and currently working towards becoming a Lead Trainer.

Pauline has a background in physiotherapy and has worked for both government and non-government organisations in the field of community health and chronic condition self -management.

Pauline has had an interest in patient self-management for many years. She first trained as a Stanford Arthritis Self Help Leader 20 years ago while working as the physiotherapist at Parks Community Health.

I have a background in chemistry and later studied homeopathy after developing a health condition in my early twenties.

As the first EPP Volunteer to be signed off as a T- Trainer by Stanford, post running Master Training in San Francisco in February 2012; Kiron truly epitomises the ethos of the Programme in the UK. The positive impact that the Programme she attended as a participant seven years prior, had on her life and her ability to manage her condition on a daily basis, created the desire in her for greater involvement.

During my career as an engineer and professional metallurgist I was involved in a 2 year preparation and writing of a Guide to Health for the Hard Metals Industry Association along with a series of health issue lectures specific to working with reactive and volatile co-compound refractory metals. This stimulated an interest in self-management particularly when I developed a long term health condition and subsequently joined a then very new Expert Patients Programme course. The course inspired me to train as a tutor and I progressed to becoming an assessor for EPP, after tutoring/assessing some 30 courses I progressed to tutor training and I am now a EPP Lead trainer with recognition as a Stanford Master Trainer.

Having a unique range of self-management and professional skills to draw upon, and my initial training in motivational interviewing, my interest was drawn towards co-authoring and developing the flexible specific needs programme called HOPE with Dr Andy Turner at Coventry University resulting in an Honorary Research Fellowship. I have delivered and developed a range of specialist HOPE courses for mental health problems, Alzheimer's, parents of children with ASD/Asperger's, ADD/ADHD, carers of people with Learning Difficulties and cancer survivors. As the principle trainer for HOPE, I have trained 16 tutors and continue a supportive role for tutors during course delivery.

After graduating as a pharmacist from University of East Anglia in 2005, I worked as a locum pharmacist for 5 months before becoming an independent community pharmacy manager seeking to embed the Healthy Living Pharmacy programme within the borough of Camden.

In May 2014, I discovered a new role as SelfCare Pharmacy Practice project lead and mental health lead at the North East London Local Pharmaceutical Committee (NEL LPC), an NHS statutory body representing 320 pharmacies across six boroughs of NEL. This new practice focusses on integrating health coaching skills to empower patients, the public and local communities to possess the confidence, skills and knowledge to self-manage their long term condition. As part of developing a new socially responsible role for community pharmacies in improving health, saving lives and reducing health inequalities, a work colleague introduced me to the CDSMP at Stanford University. I was inspired by its simplicity and effectiveness in application, by using community pharmacists and lay leaders in their communities.

Jim Phillips is a international expert on self-management. Jan's career began lecturing in Further Education, supporting young people with learning disabilities. Whilst her 6 children were growing up, she established a successful country house hotel, sourcing organic produce in Shropshire. My professional training in nursing, extensive experience of hospital, community and end-of-life care and my deep involvement in self-management have contributed to my broad skill base.

Working as a community Macmillan nurse involved taking a multi-disciplinary approach to champion best practice in person-focused care.

Moving into the field of self-management furthered opportunities to encourage people to take control of their lives. I accepted the post of Senior Trainer at the start of the EPP pilot in 2002. In this role I motivated volunteers, mobilised PCTs and voluntary organisations and mentored colleagues within the national programme. It was a joy to work within an ethos of enabling people to reach their potential.

EPP's firm adherence to Ss2Q quality assurance principles provided the framework within which I developed self-management on individual, group and service levels. My role as Business Development Manager extended my knowledge and understanding of commissioning and service development but I especially enjoy developing and delivering innovative training materials, independently and as part of a team. I am experienced in writing articles, contributing to reports, papers and national newsletters.

Being in complete support of the ethos of self-management, I am delighted to be able to support the programme as a Tutor, Assessor and more recently a Master Trainer.

During my working life, as a designer engineer and holding various technical / managerial positions I feel well equipped to deliver the Chronic Disease Self-Management Programme. Also working as a volunteer with my local cardio-vascular rehabilitation programme, and facilitating a diabetes peer to peer group, I have found yet more ways of encouraging self care.

Having been trained as an EPP Tutor in 2005 I have delivered many courses and carried out numerous assessments and believe that the programme should be delivered to a high standard and is both understandable and beneficial to all those who attend the programme.

I have been involved with lay-led self-management since the beginning. I am a Stanford University T-Trainer and Ss2Q Trainer of Lead Trainers. I led the Challenging Arthritis programme and supported voluntary organisations through the Long Term Medical Conditions Alliance (now Long-term Conditions Alliance, LTCA), to develop lay-led generic programmes using the Chronic Disease Self-Management course (CDSMC).

This pioneering work provided a blueprint for the development of the NHS Expert Patients Programme (EPP). I was a member of the Chief Medical Officer's EPP Task Force and I led the development and successful implementation of the EPP. Over a period of 5 years I provided strategic and operational leadership to the EPP, through to the establishment of the EPP as a Community Interest Company. I was at the forefront of the development of Ss2Q, the quality assurance framework for providers of Stanford University programmes.

In 2005 my contribution to the development of the knowledge base for lay led self-management was recognised with the award of an MBE. I left EPP in October 2007, since then I have worked as a freelance consultant within the field of lay led self-management. I am Chair of the Medicines Information Project Board.

I have a BA (Hons) degree in English and am a member of the Guild of Health Writers.

Following a career spanning commercial public relations, marketing and specialist journalism, I trained as a professional homeopath, since when I have focused on working in the health sector. I have edited publications for several health charities, including the Long-term Conditions Alliance (now National Voices) for whom I continue to work. My involvement with LTCA introduced me to lay led self-management, when I edited the 'Living with Long-term Illness' and 'Living Well' project reports. I was then asked to create a newsletter for the nascent Expert Patients Programme and continued to produce 'EPP Update' until it ceased publication. I also briefly acted as EPP Communications Manager to cover maternity leave, during which time I oversaw the development of the EPP CIC corporate identity.

Andy is a Senior Research Fellow at the Applied Research Centre of Health and Lifestyle Interventions, based at Coventry University. He has been evaluating lay-led, patient and carer programmes for over 10 years in the UK. He is an accredited Expert Patients Programme tutor and assessor. I joined Arthritis Care as a trainer and tutor for Challenging Arthritis Courses (Stanford supported) when the programme was quite new to the UK. I have arthritis myself and a background in social work and teaching.

I joined the DH Expert Patients Programme at its start in 2002, I have since this time helped set up administrative procedures and organise national events for EPP when government funded, I also worked as a senior tutor trainer, organising and running tutor training and support events, and recruiting tutors for the Expert Patients Programme.
